Q: Fee Schedule?

Initial visit is $485 for a 60 minute session. 

Follow up visit is $325 for a 25 minute session, $400 for a 45 minute session

Medication management/ Prescription refill is $185 for a 15 minute session

Prescription refill without visit or medication prior authorization is $75

FMLA/Disability/Emotional Support Paperwork is $200

Phone calls with family members or other providers is $75 in 20 minute increments

Q: Office schedule?

Tuesday, Wednesday and Thursday from 8:30am till 3:30pm.

Q: How can I pay ?

Sessions can be paid for by credit card.

Q: Do you take insurance?

Sorry we don’t take insurance, but a superbill will be provided upon request for insurance reimbursement.

Q: What is your cancellation policy?

You can cancel 24 hours prior to appointment or a no show fee of $100 will be charged.
